It is common clinical practice to advise patients with cryptorchidism to consider a vasectomy and subsequent long-term donor insemination (LTI) as a possible form of postoperative treatment. However, no systematic review has been conducted to critically evaluate the evidence of this form of treatment. To systematically review the evidence for the effectiveness and safety of vasectomy and LTI in the treatment of cryptorchidism. The Cochrane Controlled Trials Register, MEDLINE, and EMBASE were searched. Reference lists from all identified articles were searched for additional trials. Trials were identified that evaluated the effectiveness of vasectomy and LTI in cryptorchidism. In addition, both preclinical and clinical studies were included in the systematic review. Two reviewers extracted data independently. Two reviewers assessed the quality of the included trials and assessed the quality of evidence. Data were abstracted into a single review file. The meta-analysis results were then combined using the generic inverse variance method. Sixteen studies were included in the review. Of these, six were randomized controlled trials. The evidence for the effectiveness of vasectomy and LTI for cryptorchidism is poor. From the six randomized controlled trials, there was no significant difference between the two intervention groups in terms of postoperative recovery of the testis, paternity rate, time of recovery, risk of testicular cancer, or, in one study, the chance of a spontaneous descent of the testis.
